Privacy of Personal Information at In-Common Laboratories ("ICL")
ICL is committed to maintaining the privacy and security of health information as required by the Canadian federal Personal Information Protection and Electronic Documents Act (S.C. 2000, c. 5) and the Ontario provincial Personal Health Information Protection Act, 2004. Accordingly, ICL has developed a Privacy Policy which balances your right to have your personal and health information securely maintained and allow us to use this information on your behalf. Our Privacy Policy is based on the Canadian Standards Association Model Code for the Protection of Personal Information (CAN/CSA-Q830-96). The main issues addressed by our Privacy Policy are:

  1. The collection, use, and disclosure of your personal and health information;
  2. The retention and protection of your personal and health information;
  3. Your right of access, correction, or complete removal of your personal and health information being held by ICL on your behalf;
  4. That ICL will protect and keep secure all of your personal and health information given to us by you or your health provider in connection with any requested testing; and
  5. That ICL will provide your private information only to persons authorized by you to receive the information in order to complete the required testing and fulfil the requested services.
